Three main factors play an essential role in the development of chronic hand eczema: excessive contact with water and irritants (cumulative-toxic stress), contact with allergens, and atopic diathesis, which has a genetic component. Individual hand eczema types are identified and named according to the main catalysts involved, i.e. cumulative-toxic, contact-allergenic, or atopic hand eczema. Activities that are stressful for the skin or involve repeated, excessive contact with water or skin-irritating substances at work or home can cause damage to the skin's protective abilities and increase the chances of inflammation. This also applies to prolonged wearing of protective rubber gloves and similar materials, since sweating occurs within these gloves. Disturbance of the skin's protective barrier also facilitates penetration by allergenic substances and promotes the development of contact dermatitis. Contact allergies play a very important role in the development of hand eczema. If the hand is subjected to repeated contact with a substance that leads to an allergy, the skin reacts with signs of inflammation. Numerous people affected by hand eczema also experience skin inflammation on their feet. Often, a contact allergy to an ingredient in shoe leather treatment may be the catalyst. Contact allergies in certain types of employment are especially problematic, particularly if the work involves the handling of allergenic materials, e.g. masonry work or hairdressing.
People allergic to Balsam of Peru may experience a flare-up of hand eczema if they use or consume products that use it as an ingredient.
Severe and chronic eczema qualifies as one of the most frequent occupational illnesses. Patients should therefore be referred to an occupational-dermatological specialist as soon as possible. Patients with a history of neurodermitis, hay fever, or allergic asthma often develop hand eczema. These sicknesses reflect the individual's particular susceptibility or genetic predisposition to react over-sensitively to various environmental influences. This is described as atopy or atopy diathesis. Atopic diathesis is frequently accompanied by genetically conditioned problems with the skin's protective barriers, which causes a weakening of the skin's resistance against irritating substances and moisture, as well as easier penetration by allergens, which may lead to the development of contact allergies.
The exact causes of dyshidrosis are unknown. In 2013, a randomized, double-blind, placebo-controlled cross-over study by the University Medical Center Groningen reported that dyshydrosis outbreaks on the hands increased significantly among those allergic to house dust mites, following inhalation of house dust mite allergen.
Food allergens may be involved in certain cases. Cases studies have implicated a wide range of foods including tuna, tomato, pineapple, chocolate, coffee, and spices among others. A number of studies have implicated balsam of Peru.
Id reaction and irritant contact dermatitis are possible causes.
There are a number of different causes of skin inflammation of the hands, the interplay of which is also significant: environmental factors such as excessive water; contact with allergens or irritants; and genetic disposition. A single catalyst is seldom responsible for the development of hand eczema in patients.
According to the hygiene hypothesis, when children are brought up exposed to allergens in the environment at a young age, their immune system is more likely to tolerate them, while children brought up in a modern "sanitary" environment are less likely to be exposed to those allergens at a young age, and, when they are finally exposed, develop allergies. There is some support for this hypothesis with respect to AD. Those exposed to dogs while growing up have a lower risk of atopic dermatitis. There is also support from epidemiological studies for a protective role for helminths against AD. Likewise children with poor hygiene are at a lower risk for developing AD, as are children who drink unpasteurised milk.
In a small percentage of cases, atopic dermatitis is caused by sensitization to foods. Also, exposure to allergens, either from food or the environment, can exacerbate existing atopic dermatitis. Exposure to dust mites, for example, is believed to contribute to one's risk of developing AD. A diet high in fruits seems to have a protective effect against AD, whereas the opposite seems true for fast foods. Atopic dermatitis sometimes appears to be associated with celiac disease and non-celiac gluten sensitivity, and the improvement with a gluten-free diet indicates that gluten is a causative agent in these cases.
There is no good evidence that a mother's diet during pregnancy, the formula used, or breastfeeding changes the risk. There is tentative evidence that probiotics in infancy may reduce rates but it is insufficient to recommend its use.
People with eczema should not get the smallpox vaccination due to risk of developing eczema vaccinatum, a potentially severe and sometimes fatal complication.
The hygiene hypothesis postulates that the cause of asthma, eczema, and other allergic diseases is an unusually clean environment. It is supported by epidemiologic studies for asthma. The hypothesis states that exposure to bacteria and other immune system modulators is important during development, and missing out on this exposure increases risk for asthma and allergy.
While it has been suggested that eczema may sometimes be an allergic reaction to the excrement from house dust mites, with up to 5% of people showing antibodies to the mites, the overall role this plays awaits further corroboration.
With no particular affinity to any particular ethnic group, seen in all age groups and equally amongst males and females, the precise prevalence is not known.
About 1 in 2,000 people are affected in Sweden. Males and females appear to be affected equally.
One way to treat xerotic eczema is to avoid ing the affected area and to apply anti-itch or moisturizing lotion frequently.
A study published in 2005 found positive results from soaking the affected area in water for twenty minutes and then applying mid- to high-strength corticosteroid ointment.
There has been anecdotal evidence provided by Elaine Ingham according to which susceptibility to facial eczema in cattle is related to nutrient deficiency of forage. The experiment, which Ingham conducted, showed that by improving soil biology the forage had better nutrient qualities and was associated with an elimination of the disease.
Many contact sensitizers or irritants are known to cause contact dermatitis superimposed on nummular dermatitis. Studies have implicated nickel, cobalt, chromate, and fragrance as likely culprits. Xerosis, or dehydration of skin is also a likely cause. Infection with "Staphylococcus aureus" bacteria or "Candida" may also play a role.
Symptoms are thought to be the result of histamine being released by mast cells on the surface of the skin. Due to the lack of antigens, histamine causes the skin to swell in affected areas. If the membrane that surrounds the mast cells is too weak it will easily and rapidly break down under physical pressure, which will therefore cause an allergic-like reaction.
Symptoms can be caused or induced by

- tight or abrasive clothing
- watches
- glasses
- heat
- cold
- anything placing pressure on exposed skin
- infection
The underlying cause of dermatographism is not known, and can last for many years without relief. The condition may subside and be effectively cured; however, it is often a lifelong ailment. It is not a life-threatening disease and is not contagious.
Dermographism may occur in Mastocytosis (systemic mast cell proliferation).
Facial eczema, FE, is a disease that mainly affects ruminants such as cattle, sheep, deer, goats and South American camelids (alpaca, llamas).
It is caused by the fungus "Pithomyces chartarum" that under favorable conditions can rapidly disseminate in pastures. The fungus requires warm humid weather with night time temperatures of over 13 °C (55 °F) for several days, and litter at the bottom of the sward.
"Pithomyces chartarum" occurs worldwide but is a problem predominantly where farm animals are intensively grazed, especially in New Zealand.
The spores of the fungus release the mycotoxin sporidesmin in the gastrointestinal tract, causing a blockage in the bile ducts that leads to injury of the liver. Bile, chlorophyll and other waste products consequently build up in the bloodstream causing photo sensitivity of the skin especially that exposed to direct sunlight. This in turn causes severe skin irritation that the animal attempts to relieve by rubbing its head against available objects, resulting in peeling of the skin.
The large family of fungi that produce mycotoxins, of which sporidesmin is one, live mainly on ryegrasses and can cause significant problems in grazing animals. Sporidesmin can lower an animals immunity and affect total production in farm animals, and, when taken in larger quantities, can result in death.
The clinical symptoms of FE are distressing: restlessness, frequent urination, shaking, persistent rubbing of the head against objects (e.g. fences, trees etc.), drooping and reddened ears, swollen eyes, and avoidance of sunlight by seeking shade. Exposed areas of skin develop weeping dermatitis and scabs that can become infected and attractive to blow-fly causing myiasis.
Rosin, the material commonly used to wax string instruments is known to cause allergic contact dermatitis in musicians. Nickel, a metal found in musical instruments causes allergic contact dermatitis on the fingers and hands of string instrumentalists and in the lip and neck of wind instrumentalists. Wind instrumentalists with lip and neck infection should switch to gold or plastic mouthpieces if allergic dermatitis occurs. (R)-4-methoxydalbergione present in rosewood may cause allergic contact dermatitis in violinists. Cane reed (causing chelitis in saxophone players), propolis (a wax used to close structural gaps in musical instruments), paraphenylenediamine (used to polish musical instruments) and potassium dichromate (tanning agent to the skin of the harp) also cause allergic contact dermatitis in musicians.
The prevalence of nummular dermatitis in the United States is approximately 2 per 1,000. It is considered a disease of adulthood, for it is rare in children.
Any dermatitis may heal leaving pale skin, as may excessive use of corticosteroid creams used to treat episodes of eczema. The hypopigmentation is due to both reduced activity of melanocytes with fewer and smaller melanosomes.
The condition is most often seen in children between the ages of 3 and 16 years and is more common in males than females. However adults can also suffer from this disease.
It may occur more frequently in lighter-skinned patients, but is more apparent in those with darker complexions.
Up to a third of US school children may at some stage have this condition. Single-point prevalence studies from India have shown variable rates from 8.4%,
to 31%.
Other studies have shown prevalence rates in Brazil of 9.9%,
Egypt 13.49%,
Romania 5.1%,
Turkey 12% where higher rates were seen in those with poor socioeconomic conditions,
and just 1% in school children in Hong Kong.
In adults, the prevalence of IgE sensitization to allergens from house dust mite and cat, but not grass, seem to decrease over time as people age. However, the biological reasons for these changes are not fully understood.
Other rashes that occur in a widespread distribution can look like an id reaction. These include atopic dermatitis, contact dermatitis, dyshidrosis, photodermatitis, scabies and drug eruptions.
Dermatographic urticaria (also known as dermographism, dermatographism or "skin writing") is a skin disorder and one of the most common types of urticaria affecting 2–5% of the population.
The patches of pityriasis alba may last from 1 month to about one year, but commonly on the face last a year. However it is possible that the white patches may last for more than 1 year on the face.
A common cause of lick granuloma appears to be psychological, related to stress, anxiety, separation anxiety, boredom, or compulsiveness. Lick granulomas are especially seen in large active dogs left alone for long periods of time. It is often considered to be a form of canine obsessive-compulsive disorder.
But other less common causes include bacterial or fungal infections, trauma causing nerve damage, allergies, or joint disease. Hot spots may also lead to the formation of lick granulomas.
Many large breed dogs appear to be predisposed, as well as golden retrievers and other bird dogs. Most of the dogs with the condition are over five years of age. Seventy percent of the time it occurs on one of the dog's left legs. Arthritic and mobility problems in older dogs give them more time to lick and over-groom themselves.
Frequent formations of lick granulomas in the same area due to the constant licking will cause hardening, callous formation, hair loss (the hair may stop growing back), and hyperpigmentation to that area. The condition becomes a vicious cycle – erosion of the skin from licking leads to pain and itching, which leads to more licking. Lick granulomas sometimes become infected with bacteria, causing abscessed areas or fistulous tracts (furuncles).
The intense contact between a musical instrument and skin may exaggerate existing skin conditions or cause new skin skin conditions. Skin conditions like hyperhidrosis, lichen planus, psoriasis, eczema, and urticaria may be caused in instrumental musicians due to occupational exposure and stress. Allergic contact dermatitis and irritant contact dermatitis are the most common skin conditions seen in string musicians.
Xerotic eczema (also known as "Eczema craquelé", "Pruritus hiemalis", "Asteatotic eczema", "Winter itch", "Desiccation dermatitis," and "Winter eczema") is a form of eczema that is characterized by changes that occur when skin becomes abnormally dry, itchy, and cracked. Lower legs tend to be especially affected, although it can appear in the underarm area as well.
Xerotic eczema is common in elderly people, though it is not uncommon for people in their 20s. It can appear in red, bumpy, pimple-like irritations. Shaving can cause it to become inflamed.
Opportunistic infections (infections that are caused by a diminished immune system) are frequent. Fungus from an athlete's foot infection can spread to the groin through clothing. Tight, restrictive clothing, such as jockstraps, traps heat and moisture, providing an ideal environment for the fungus.
The type of fungus involved is usually "Trichophyton rubrum". Some other contributing fungi are "Candida albicans", "Trichophyton mentagrophytes" and "Epidermophyton floccosum".
