The major objectives of the topical treatment is to kill any bacteria or fungi and to reduce the inflammation.
Systemic therapy is only for the most severe cases under the supervision of a veterinarian.
Rainscald normally heals on its own, however as the condition can spread to involve large areas, prompt treatment is recommended. Although some cases can be severe, most rain scald is minor and can be easily and cheaply treated at home naturally.
First groom the affected parts carefully, to remove any loose hair. Be extremely gentle, the area is very sore itchy and horses will very quickly get fidgety. Next shampoo the area, use warm water and a soft cloth or brush, and massage the lather through the coat as much as the horse will tolerate. It is best to use Neem shampoo here, as this will treat as well as clean, but any mild shampoo is fine. Remove as much water as possible and dry the horse off, either use a hair drier or let him/her stand in the sun until completely dry. It is important not to let the horse roll! The rain scald bacteria may be picked up from the soil.
When the horse is completely dry, gently brush off any more loose hair. Next apply a salve or cream containing a high percentage of neem oil, or even pure neem oil, to liberally coat the affected area. Rub it in using fingertips, massage the area as much as the horse will tolerate. This will be very greasy. Smooth the hair back down and apply a rug to keep the horse dry, this prevents the neem being washed off as well as protecting from more dampness issues. Turn the horse out as normal.
Check it every day, and reapply the neem salve/cream if it seems to have dried away. The area should remain greasy with neem. Every 2–3 days or so, go through and scrape/pick off as much of the scabs as possible without upsetting the horse or making it bleed, then reapply the neem. Typically there will be improvement in a few days, and in a week there'll be some sign of new hair growing back. More severe rain scald may take longer.
Once all the scabs are gone and there is new hair fuzz growing in all over, use neem shampoo to clean the area of greasy residue, and dry well. Keep the horse covered for some time after rain scald has been treated, particularly in wet weather. Do not allow the skin to remain damp. It is advisable to shampoo the horse after riding or exercising, to remove sweat, which may encourage rain scald conditions, and make sure the coat is completely dry afterwards.
This treatment works in many ways. First, shampooing cleans the area of any contaminants, remove a lot of loose hair and scabs, and the rubbing stimulates the circulation. The neem is an antifungal agent, and works to eliminate the bacteria that cause the infection. It soothes the irritation in the area, and its greasiness provides the ideal environment for the raw skin to heal and grow new hair. It also helps to soften and lift the scabs. The new hair cannot grow in until those scabs are removed from the surface, but they are very painful to pick and remove, and most horses are intolerant of this procedure. After the neem has soaked into these scabs they will come away much more freely, and soon new hair will grow through.
In conventional treatment, scabs are softened with benzoyl peroxide and chlorhexidine and removed in order to speed the healing process. In severe or chronic cases, penicillin and streptomycin are injected into the horse to kill the bacteria.
Typically the disease is not life-threatening, nor does it impact the welfare of the horse, so treatments are more for the owner's sake of mind and cosmetic appeal of the animal.
In order to prevent rainscald, it is important to stop the spread of the bacteria. Tick and insect control is an effective way to stop the spread of the bacteria from one animal to another. As well, separating infected animals will help to stop the spread of the bacteria. Keeping the animal in a dry, well-ventilated area out of the rain and wet conditions will stop the bacteria from growing. This dry environment includes dry ground as well as dry air.
Treatment depends on many factors, such as the age of horse, severity of symptoms and duration of infection. As long a horse is eating and drinking, the infection must run its course, much like a common cold virus. Over time a horse will build up enough antibodies to overtake and fight the disease. Other treatment options can be applying heat packs to abscesses to help draw it to the surface and using drawing salves such as Ichthammol. A blood test or bacterial cultures can be taken to confirm the horse is fighting Pigeon Fever. Anti-inflammatory such as phenylbutazone can be used to ease pain and help control swelling. Treating Pigeon Fever with antibiotics is not normally recommended for external abscesses since it is a strong bacterium that takes extended treatment to kill off and to ensure it does not return stronger. However, if the abscesses are internal then antibiotics may be needed. Consulting a veterinarian for treatment is recommended. Making the horse comfortable, ensuring the horse has good food supply and quality hay will help the horse keep their immune system strong to fight off the infection. Once the abscess breaks or pops, it will drain for a week or two. During this time keeping the area clean, applying hot packs or drawing salves will help remove the pus that has gathered in the abscess.
This bacterium is present in soil and is transmitted to horses through open wounds, abrasions or mucous membranes.
Surgical drainage is usually indicated for prostatic abscesses and septic arthritis, may be indicated for parotid abscesses, and is not usually indicated for hepatosplenic abscesses. In bacteraemic melioidosis unresponsive to intravenous antibiotic therapy, splenectomy has been attempted, but only anecdotal evidence supports this practice.
Prior to 1989, the standard treatment for acute melioidosis was a three-drug combination of chloramphenicol, co-trimoxazole and doxycycline; this regimen is associated with a mortality rate of 80% and is no longer be used unless no other alternatives are available. All three drugs are bacteriostatic (they stop the bacterium from growing, but do not kill it) and the action of co-trimoxazole antagonizes both chloramphenicol and doxycycline.
"Warm water immersion foot" is a skin condition of the feet that results after exposure to warm, wet conditions for 48 hours or more and is characterized by maceration ("pruning"), blanching, and wrinkling of the soles, padding of toes (especially the big toe) and padding of the sides of the feet.
Foot maceration occur whenever exposed for prolong periods to moist conditions. Large watery blisters appear which are painful when they open and begin to peel away from the foot itself. The heels, sides and bony prominences are left with large areas of extremely sensitive, red tissue, exposed and prone to infection. As the condition worsens, more blisters develop due to prolonged dampness which eventually covers the entire heel and/or other large, padded sections of the foot, especially the undersides as well as toes. Each layer in turn peels away resulting in deep, extremely tender, red ulcerations.
Healing occurs only when the feet are cleansed, dried and exposed to air for weeks. Scarring is permanent with dry, thin skin that appears red for up to a year or more. The padding of the feet returns but healing can be painful as the nerves repair with characteristics of diabetic neuropathy. Antibiotics and/or antifungal are sometimes prescribed.
Foot immersion is a common problem with homeless individuals wearing one pair of socks and shoes for extensive periods of time, especially wet shoes and sneakers from rain and snow. The condition is exacerbated by excessive dampness of the feet for prolonged periods of time. Fungus and bacterial infections prosper in the warm, dark, wet conditions and are characterized by a sickly odor that is distinct to foot immersion.
When proper treatment is provided for patients with rat-bite fever, the prognosis is positive. Without treatment, the infection usually resolves on its own, although it may take up to a year to do so. A particular strain of rat-bite fever in the United States can progress and cause serious complications that can be potentially fatal. Before antibiotics were used, many cases resulted in death. If left untreated, streptobacillary rat-bite fever can result in infection in the lining of the heart, covering over the spinal cord and brain, or in the lungs. Any tissue or organ throughout the body may develop an abscess.
Ticks should be removed promptly and carefully with tweezers and by applying gentle, steady traction. The tick's body should not be crushed when it is removed and the tweezers should be placed as close to the skin as possible to avoid leaving tick mouthparts in the skin; mouthparts left in the skin can allow secondary infections. Ticks should not be removed with bare hands. Hands should be protected by gloves and/or tissues and thoroughly washed with soap and water after the removal process.
A match or flame should not be used to remove a tick. This method, once thought safe, can cause the tick to regurgitate, expelling any disease it may be carrying into the bite wound.
No specific treatment for CTF is yet available. The first action is make sure the tick is fully removed from the skin, then acetaminophen and analgesics can be used to help relieve the fever and pain. Aspirin is not recommended for children, as it has been linked to Reye’s syndrome in some viral illnesses. Salicylates should not be used because of thrombocytopenia, and the rare occurrence of bleeding disorders. People who suspect they have been bitten by a tick or are starting to show signs of CTF should contact their physicians immediately.
On the basis of the laboratory evidence and case reports, amphotericin B has been the traditional mainstay of PAM treatment since the first reported survivor in the United States in 1982.
Treatment has often also used combination therapy with multiple other antimicrobials in addition to amphotericin, such as fluconazole, miconazole, rifampicin and azithromycin. They have shown limited success only when administered early in the course of an infection. Fluconazole is commonly used as it has been shown to have synergistic effects against naegleria when used with amphotericin in-vitro.
While the use of rifampicin has been common, including in all four North American cases of survival, its continued use has been questioned. It only has variable activity in-vitro and it has strong effects on the therapeutic levels of other antimicrobials used by inducing cytochrome p450 pathways.
In 2013, the two most recent successfully treated cases in the United States utilized drug combinations that included the medication miltefosine as well as targeted temperature management to manage brain swelling that is secondary to the infection. As of 2015 there were no data on how well miltefosine is able to reach the central nervous system. As of 2015 the U.S. CDC offered miltefosine to doctors for the treatment of free-living ameobas including naegleria.

Some authorities recommend the use of azithromycin.
African tick bite fever is usually mild, and most patients do not need more than at-home treatment with antibiotics for their illness. However, because so few patients with this infection visit a doctor, the best antibiotic choice, dose and length of treatment are not well known. Typically doctors treat this disease with antibiotics that have been used effectively for the treatment of other diseases caused by bacteria of similar species, such as Rocky Mountain Spotted Fever.
For mild cases, people are usually treated with one of the following:
- doxycycline
- chloramphenicol
- ciprofloxacin
If a person has more severe symptoms, like a high fever or serious headache, the infection can be treated with doxycycline for a longer amount of time. Pregnant women should not use doxycycline or ciprofloxacin as both antibiotics can cause problems in fetuses. Josamycin has been used effectively for treatment of pregnant women with other rickettsial diseases, but it is unclear if it has a role in the treatment of ATBF.
"Trench foot " is a medical condition caused by prolonged exposure of the feet to damp, unsanitary, and cold conditions. The use of the word "trench" in the name of this condition is a reference to trench warfare, mainly associated with World War I. Affected feet may become numb, affected by erythrosis (turning red) or cyanosis (turning blue) as a result of poor vascular supply, and feet may begin to have a decaying odour due to the possibility of the early stages of necrosis setting in. As the condition worsens, feet may also begin to swell. Advanced trench foot often involves blisters and open sores, which lead to fungal infections; this is sometimes called tropical ulcer (jungle rot).
If left untreated, trench foot usually results in gangrene, which can cause the need for amputation. If trench foot is treated properly, complete recovery is normal, though it is marked by severe short-term pain when feeling returns. As with other cold-related injuries, trench foot leaves sufferers more susceptible to it in the future.
While obviously preventable by staying away from rodents, otherwise hands and face should be washed after contact and any scratches both cleaned and antiseptics applied. The effect of chemoprophylaxis following rodent bites or scratches on the disease is unknown. No vaccines are available for these diseases.
Improved conditions to minimize rodent contact with humans are the best preventive measures. Animal handlers, laboratory workers, and sanitation and sewer workers must take special precautions against exposure. Wild rodents, dead or alive, should not be touched and pets must not be allowed to ingest rodents.
Those living in the inner cities where overcrowding and poor sanitation cause rodent problems are at risk from the disease. Half of all cases reported are children under 12 living in these conditions.
Proper dietary management will prevent most cases of milk fever. This generally involves close attention to mineral and fiber levels in the diet prior to calving, as well as improving cow comfort to eliminate other problems that may interfere with appetite and so trigger hypocalcemia.
Oral administration of a dose of a calcium salt in a gel has been advised by some veterinarians.
An orally administered bolus containing a much higher concentration of calcium than the injectable solutions can also be given so long as the cow is standing or sitting up. If the cow is lying 'flat out' then immediate intravenous therapy is required to avoid death.
Treatment generally involves calcium injection by intravenous, intramuscular or subcutaneous routes. Before calcium injection was employed, treatment comprised inflation of the udder using a pneumatic pump. Inflation of the udder worked because the increased pressure created in the udder pushed the calcium in the udder back into the bloodstream of the cow.
Intravenous calcium, though indicated in many cases, is potentially fatal through "heart blockade", or transient high calcium levels stopping the heart, so should be administered with care.
Cows are to be fed jaggery along with the lime water mixture.
In unclear cases of downer cows, intravenous calcium injection can lead to diagnosis. The typical reaction will be a generalized tremor of the skeletal muscles, and sometimes cardiac arrhythmia. Defecation, urination and eructation are frequent during the treatment, due to pharmacological effect of calcium on the smooth muscles.
The primary treatment strategy is to eliminate or discontinue the offensive agent. Supportive therapy, such as ice packs, may be provided to get the body temperature within physiologic range. In severe cases, when the fever is high enough (generally at or above ~104° F or 40° C), aggressive cooling such as an ice bath and pharmacologic therapy such as benzodiazepines may be deemed appropriate.
Effective antibiotics include penicillin G, ampicillin, amoxicillin and doxycycline. In more severe cases cefotaxime or ceftriaxone should be preferred.
Glucose and salt solution infusions may be administered; dialysis is used in serious cases. Elevations of serum potassium are common and if the potassium level gets too high special measures must be taken. Serum phosphorus levels may likewise increase to unacceptable levels due to kidney failure.
Treatment for hyperphosphatemia consists of treating the underlying disease, dialysis where appropriate, or oral administration of calcium carbonate, but not without first checking the serum calcium levels (these two levels are related). Administration of corticosteroids in gradually reduced doses (e.g., prednisolone) for 7–10 days is recommended by some specialists in cases of severe hemorrhagic effects. Organ-specific care and treatment are essential in cases of kidney, liver, or heart involvement.
When the appearance is caused by heat, the lesion is usually completely reversible within a few weeks if the smoking habit is stopped. This is the case even if the condition has been present for decades. Without stopping smoking, spontaneous remission of the lesion is unlikely. If the lesion persists despite stopping smoking, this is usually then considered to be a true leukoplakia rather than a reactionary keratotis, and may trigger the decision to carry out a biopsy to confirm the diagnosis. Since this condition almost always develops in the setting of long term heavy smoking, it usually indicates the need for regular observation for cancers associated with smoking, e.g. lung cancer.
Canakinumab has been approved for treatment of HIDS and has shown to be effective. The immunosuppressant drugs etanercept and anakinra have also shown to be effective. Statin drugs might decrease the level of mevalonate and are presently being investigated. A recent single case report highlighted bisphosphonates as a potential therapeutic option.
There are no treatment modalities for acute and chronic chikungunya that currently exist. Majority of treatment plans use supportive and symptomatic care like analgesics for pain and anti-inflammatories for inflammation caused by arthritis. In acute stages of this virus, rest, antipyretics and analgesics are used to subside symptoms. Most use non-steroidal anti-inflammatory drugs (NSAIDs). In some cases, joint pain may resolve from treatment but stiffness remains.
Treatment of mild metal fume fever consists of bedrest, keeping the patient well hydrated, and symptomatic therapy (e.g. aspirin for headaches) as indicated. In the case of non-allergic acute lung injury, standard or recommended approaches to treatment have not been defined.
The consumption of large quantities of cow's milk, either before or immediately after exposure is a traditional remedy. However, the United Kingdom Health and Safety Executive challenges this advice, warning, "Don’t believe the stories about drinking milk before welding. It does not prevent you getting metal fume fever."
